What is Apexification?
Apexification is a specialized endodontic procedure that saves teeth whose roots never finished developing. At Williamsburg Endodontics in Brooklyn, we most often perform apexification for children, teenagers, and young adults whose permanent teeth were injured or infected before the root tips had time to close naturally.
A permanent tooth erupts before its root is complete. For several years after a tooth appears, the root continues to lengthen and the opening at its tip, the apex, gradually narrows and closes. If deep decay or a traumatic injury kills the pulp during this window, root development stops. The result is a tooth with thin root walls and a wide open apex, and that open apex is a serious problem: a conventional root canal filling cannot be sealed against an opening that has no bottom.
Apexification solves this by creating an artificial barrier at the root tip. We clean and disinfect the canal, then place a biocompatible material, typically mineral trioxide aggregate, known as MTA, at the apex. This material stimulates the formation of a hard tissue barrier and gives the canal a solid floor against which a permanent root canal filling can finally be sealed.
What can you expect during apexification at our Williamsburg office?

Treatment is performed under local anesthesia and typically requires two or more visits: one to disinfect the canal and place the MTA barrier, and a subsequent visit to confirm the seal and complete the root canal filling. Between visits, a temporary filling protects the tooth completely.
Follow up matters more with apexification than with almost any other endodontic procedure. We monitor the tooth with periodic imaging to confirm the barrier is stable and the surrounding bone remains healthy.
